  • Simula sa September 1, 2020:
  1. Lahat ng Prenatal Check-up ng mga Bagong Pasyente sa Dr. Jose Fabella Memorial Hospital (DJFMH) Out-Patient Department (OPD) ay kailangan ipa-schedule.
  2. Magsisimula na rin magtanggap ng Video Consultations ng low-risk na buntis ang DJFMH.
  • Para magpa-schedule ng REGULAR na prenatal:
  1. Mag-text sa 0931-2144190 at ibigay ang mga sumusunod na impormasyon:
    • Mensahe: maaaring alinman sa mga sumusunod
      • Magpa-schedule ng prenatal check-up
      • Magkansela ng schedule ng prenatal check-up
      • Magpa-reschedule ng prenatal check-up
      • Iba pang katanungan (tukuyin)
    • Buong Pangalan (Una, Gitna, Apelyido)
    • Edad
    • Pang-ilang Pagbubuntis
    • Ilang Buwan na ang Pagbubuntis
    • Ibang Sakit Maliban sa Pagbubuntis
    • Telepono (cellphone o landline)
  2. Hintayin ang kasagutan at i-save ang nakatalagang schedule para sa iyong pagkonsulta. Kung hindi ka pwede sa nakatalagang schedule, tumawag o i-text ang impormasyong nakasulat sa itaas sa parehong numero para humingi ng bagong schedule.
  3. Pumunta sa DJFMH Out-Patient Department (OPD) ng maaga sa nakatakdang araw ng konsultasyon at kumuha ng numero sa pagpila sa OPD Information Area. Huwag kalimutan magsuot ng mask at face shield. Kung kinakailangan mong magkansela ng appointment, i-text ang impormasyong nakasulat sa itaas sa parehong numero para humingi ng bagong schedule.
  • Para magpa-schedule o magtanong tungkol sa Video Consultation (VC):
  1. I-text or ipadala ang mga sumusunod na impormasyon sa 0931-2144190 o sa Facebook page ng “OPD Fabella” o i visit ang aming website sa http://fabella.doh.gov.ph/.
    • Mensahe: maaaring alinman sa mga sumusunod
      • Magpa-schedule ng Video Consultation (VC)
      • Magpa-kansela ng VC appointment
      • Magpalit ng schedule ng VC appointment
      • Iba pang katanungan (tukuyin)
    • Buong Pangalan ng Pasyente
    • Pangalan ng Doktor o Team (kung lumang pasyente). Ilagay ang “Prenatal Check-up” (kung bagong pasyente)
    • Gustong Petsa at Oras ng Video Consultation
    • Cellphone Number o Email ng Pasyente
  2. Hintayin ang kasagutan. Kung hindi ka pwede sa video consultation, magpa-schedule para sa regular na prenatal (tingnan sa itaas).
  3. Kung may kumpirmadong appointment na: i-save ang petsa at oras nito at ang link na ipinadala sa iyo.
    • Kung bagong pasyente: i-download at basahin ang Pahintulot Para sa Telehealth Consultation na makikita sa Facebook page ng “OPD Fabella”. Kung sumasang-ayon sa mga nakasulat dito, pirmahan ang consent form (o ibigay ito sa iyong magulang o guardian para pirmahan) at ipadala ito sa This email address is being protected from spambots. You need JavaScript enabled to view it. bago ang nakatakdang schedule ng VC.
    • Kung kinakailangan magpadala ng laboratory/ultrasound/x-ray/iba pang resulta na may kaugnayan sa karamdaman ng pasyente, i-scan o kunan ng litrato ang mga ito at ipadala sa This email address is being protected from spambots. You need JavaScript enabled to view it. para maikabit sa tsart ng pasyente.
  • Mga hakbang sa pag-log-in para sa Video Consultation:
  1. Mag-log-in ng 5 minuto bago ang nakatakdang oras sa araw ng konsultasyon. Buksan ang computer, smart phone, o tablet.
  2. Kumonekta sa maganda at matatag na internet.
  3. I-type ang link na ipinadala sa iyo sa browser ng computer. Pwedeng gumamit ng Chrome, Firefox, o Safari.
  4. Payagan ang browser na gamitin ang webcam at mikropono ng computer.
  5. Nasa webpage ka na ng platform. I-type ang iyong pangalan o ang iyong Patient Health Card (PHC) number at i-click ang “check in”.
  6. Nasa Waiting Room ka na ng doktor. Hintayin ang doktor na simulan ang iyong konsultasyon.

  • Starting September 1, 2020:
  1. All OB Consultations or Prenatal Visits of New Patients at the Dr. Jose Fabella Memorial Hospital (DJFMH) Out-Patient Department (OPD) shall be scheduled.
  2. Video Consultations of selected number of low-risk or uncomplicated pregnant patients shall also be accepted.
  • To schedule an IN-PERSON prenatal visit:
  1. Text 0931-2144190 and give the following information:
    • Message: could be any of the following
      • Scheduling of prenatal check-up
      • Cancellation of prenatal check-up schedule
      • Re-scheduling of prenatal check-up schedule
      • Other inquiries (specify)
    • Full Name (First, Middle, Last)
    • Age
    • Number of Pregnancies
    • Length of Current Pregnancy
    • Other Illnesses
    • Contact Number (mobile or landline)
  2. Wait for the response and save the assigned schedule for your visit. If you are not available on the assigned schedule, you may call or text the same information written above to the same number to request for another schedule.
  3. Go to the DJFMH Out-Patient Department (OPD) early on the day of your appointment and get a queue number at the OPD Information Area. Do not forget to wear mask and face shield. If you need to cancel an appointment, text also the same information to the same number to cancel an appointment.
  • To schedule or inquire about Video Consultation (VC):
  1. Text or send the following information to 0931-2144190 or the Facebook page of “OPD Fabella” or visit our website at http://fabella.doh.gov.ph/.
    • Message: could be any of the following
      • Scheduling of Video Consultation (VC)
      • Cancellation of confirmed VC appointment
      • Re-scheduling of VC appointment
      • Other inquiries (specify)
    • Full Name of Patient
    • Name of Physician or Team (if old patient). Write “Prenatal Check-up” (if new patient).
    • Preferred Date and Time of Video Consultation
    • Patient’s Mobile Number or email or other contact details
  2. Wait for the response. If you are not approved for video consultation, schedule for an in-person prenatal visit (see above).
  3. If with confirmed appointment: save the date and time and the link sent to you.
    • If new patient: download and read the Informed Consent for Telehealth Consultation from the Facebook page of “OPD Fabella”. If you agree with what is written here, sign the consent form (or have it signed by your parent or guardian) and send it back to This email address is being protected from spambots. You need JavaScript enabled to view it. before the schedule of the VC.
    • If you need to sent laboratory/ultrasound/x-ray/other results related to the patient’s condition, scan or take photos of these and send it also to This email address is being protected from spambots. You need JavaScript enabled to view it. to be attached to the patient’s chart.
  • Instructions for logging-in during Video Consultation:
  1. Log-in at least 5 minutes before the assigned schedule. Open the computer, smart phone, or tablet.
  2. Connect to a good internet access.
  3. Enter the link that was sent to you into the browser. You may use Chrome, Firefox, Safari.
  4. Allow the browser to use the computer’s webcam and microphone.
  5. You are now in the platform webpage. Type your name or your Patient Health Card (PHC) number into the box and click “check in”.
  6. You are now in the doctor’s Waiting Room. Wait for the doctor to start the consultation.

HOSPITAL MEMORANDUM No. 06 s. 2020

SUBJECT: Hospital Policy During the Declaration of Health Emergency Effective Immediately

 

In the interest of the service, the following will be strictly implemented during the Declaration of Health Emergency:

1. Limit visiting privileges  - Visiting hours shall remain at 10:00 am to 12:00 noon and 4:00 to 6:00 pm daily. However, each patient shall be allowed 1 visitor per day, on either visiting period (AM or PM) only. Watchers and visitors will be requested to vacate the waiting area at 7:00 am every day for cleaning of the area and may re-enter the hospital premises through the triage for temperature check;

 

2. Limit check-up at the OPD:

a. Advise prenatal checkup follow-up after every 2 - 3 months for low risk patients;

b. Well baby checkup is temporarily suspended. Advise parents on discharge to do well baby check-up nearby health centers;

c. Internal Medicine will manage medical problems of Ob-Gyn patients only. 

 

3. Personalized Care - Doctors are advised to provide cellphone number for telephone consult and appointments. Each patient should be able to identify her personal doctor;

 

4. Private Practice - Admission of private patients is temporarily suspended.

LIVE LOCATION of Buses in the DOTr FREE RIDE FOR HEALTH WORKERS PROGRAM in Metro Manila can Now be Viewed Online
 
Frontliners can now see NOT JUST THE ROUTES of buses, but also the LOCATION OF BUSES ALONG EACH ROUTE. With this added service, frontliners will now be able to check online when there is a bus approaching, and a bus’s estimated time of arrival
 
Currently, 80 out of the 150 buses being used for the 20 routes of the DOTr Free Ride for Health Workers Program have been installed with GPS LOCATION TRACKER DEVICES, and the number of buses with GPS location trackers is expected to increase in the coming days.
 
This added service is brought by DOTr in partnership with @PLDT Enterprise and Vectras, Inc.
